Izanagi-no Okami can only be obtained by fusing Izanagi, Sandman, Nata Taishi, Girimehkala, Norn, Okuninushi, Orthrus, Kartikeya, Mithra, Tzitzimitl, Cu Chulainn, Legion on the second playthrough after completing the true ending on the first playthrough.

In P4G, that's Izanagi-no-Okami, and after it emerges to help you during the battle against Izanami, in New Game Plus, you can fuse it and add it to your team. But in order to fuse Izanagi-no-Okami, you'll need to first beat P4G, assemble a dozen ingredient Personas, and grind pretty hard for EXP.
